Responsibilities

The Pharmacist – clinics provider is responsible for managing individual patients and patient populations with complex and/or multiple medication care needs. Through skilled drug management techniques, the pharmacist – clinics provider works with the patient and members of the healthcare team to achieve optimal therapeutic outcomes. This job assimilates and applies evidence-based information in the design, implementation and evaluation of treatment plans across the continuum of the patient health care needs. In addition to providing patient care, this position is responsible for advancing the profession of pharmacy and patient care within Virginia Mason Medical Center.

This is a multiclinic float position covering primary care and anticoagulation management.

Qualifications

  • This position requires a Pharm D or B.S. in Pharmacy with relevant practice experience and a current Washington state licensure as a Pharmacist.
  • Post-graduate Year – 1 residency, or more than 3 years’ experience as a pharmacy provider as part of a physician group clinic.
  • Excellent drug management skills, ability to work collaboratively in team setting, experience with using electronic medical records, and demonstrated expertise in prescribing drug therapy under protocol also required.
  • Must meet requirements for ancillary provider credentialing at Virginia Mason.
  • Experience in managing a broad range of complex, medication intensive patients in an ambulatory/clinic setting highly preferred..
  • Specialty post-graduate year-2 residency training in ambulatory/clinical care preferred.
