Position Summary
The Pharmacist is responsible for providing pharmacy services to patients, associated facilities, and providers. Pharmacists must consult with patients, nurses and physicians on the therapy of the patients and to confirm the correct medication and dosing. The pharmacist oversees the job of the Pharmacy Technicians. Pharmacists must achieve sales and profit targets established for the pharmacy unit and monitor and control all established quality assurance standards. Pharmacists must embrace and live OptioRx core values.
Floater Pharmacists will be expected to travel between stores in Torrance and Santa Monica, two days a week.
Essential Functions
1. Evaluates physician medication orders for appropriateness of drug, dosage, potential interactions, and route of administration. Makes recommendations for therapy changes to the physician and facility staff as appropriate.
2. Maintains patient/resident database to include appropriate current and past medical history, appropriate clinical and laboratory data, and concurrent medication (including over-the-counter and home remedies).
3. Represent the pharmacy in a professional and courteous manner in all job-related interactions.
4. Maintain positive relationship with patients, health care providers and team members of the facilities.
5. Collaborates with the healthcare team to provide clinical and technical support as needed.
6. Assists facility/agency staff personnel with procedures relating to safe and effective methods of procuring, maintaining, administering, and disposing of drugs.
7. Oversees and collaborates with hospice, medical staff, pharmacy staff, quality assurance, nursing staff and other departments to evaluate the quality of pharmaceutical care services. Helps ensure compliance with Clinical Quality Indicators including CMS Measures and The Joint Commission Medication Management Standards. Establishes and maintains pharmacy policies, procedures, requirements, and standards in accordance with federal and state laws and standards of practice.
8. Oversee pharmacy operations through each respective pharmacy team member, ensuring compliance with State and Federal laws, as well as conformity with OptioRx Policies and Procedures.
9. Assessing customer feedback and using your creativity to establish, improve, and refine services.
10. Resolve pharmacy patient concerns and complaints.
11. Hold unquestionable level of personal and professional integrity, ethics and interpersonal relationships.
12. Supervise technicians in dispensing medications and patient support.
13. Help to determine ways to reduce costs without sacrificing customer satisfaction.
14. Report product incidents as required in accordance with pharmacy policy.
15. Perform other duties as assigned by supervisor.
Qualifications
Education and/or Training:
a. Bachelor of Science in Pharmacy or Doctor of Pharmacy Degree Pharmacy
b. HQAA/ACHC or similar accreditation training a plus
Professional Experience:
a. Experience in customer service, leadership
b. Ability to balance multiple projects and adapt to new issues/assignments as business needs change
c. Operates with the highest level of ethics, compliance, professionalism and integrity
d. Experience in core elements of pharmacy and business operations, including purchasing, customer service, financial management, operational excellence, clinical programs, and inventory control
e. Experience in hospice and long term care pharmacy
f. Experience hiring, training, and developing teams
Licenses/Certifications:
a. Valid and active pharmacist license in corresponding state(s)
Specialized Skills:
a. 5+ year experience in the hospice and long-term care environment
b. Previous experience collaborating with hospice and assisted living facilities, fostering and maintaining relationships with DoN, medical director, and nursing team
c. Clinical understanding of palliative care, hospice assessment tools, and treatment formularies
d. Understanding of the hospice, assisted living, group home and other alternative long-term care industry sector preferred
e. Experience with IDT meetings and partnering with Hospice facilities on licensing and compliance. Extensive knowledge in third party billing (Medicare, Medicaid, commercial PBMs) and corresponding residence code, pharmacy service type, diagnosis code, etc. and ability to interpret classification for non-covered services and products
f. Ability to organize and plan the activities of subordinates, and to delegate appropriately
g. Organizational skills sufficient to maintain consistently accurate records
h. Ability to evaluate options and to make efficient decisions
Technical Skills:
a. High degree of proficiency MS Office Suite (Excel, Word, PPT), Outlook & Internet applications
b. Must have excellent communication skills, both written and verbal, and experience with communication at an executive level.
c. Strong analytical, prioritizing, interpersonal, problem solving, and planning skills
Successfully pass background check prior to employment

Pay: $60.00 - $70.00 per hour
